Documents of proof to provide with certain forms
Some of our forms concerning public-sector pension plans (RREGOP,
PPMP, etc.) must be sent with proof. The following documents are accepted.
Information about the documents provided proof
When documents provided as proof are necessary, you can provide clear and legible copies. If necessary, we may request the original documents or certified true copies that will be sent back to you upon request. Generally, Québec residents do not need to provide proof.
Documents provided as proof of birth
When proof is required, you can provide one of the following documents:
- Birth certificate
- Copy of an act of birth
- Certificate of civil status
- Attestation of civil status
- Copy of an act of marriage
- Copy of an act of death
- Valid Canadian passport
You can also provide, among the following documents, two different documents on which the same date of birth is indicated:
- Québec health insurance card
- Québec driver's licence
- Extracts from public records (employment insurance or census)
- Medical or hospital file extracts
- Military archives extracts
- Extracts from corporate records
- Identity card issued by a public body
When proof is required, you can provide one of the following documents:
- Semi-authentic birth certificate issued by the
Directeur de l'état civil
- Certified copy of the act of birth issued by the country or province of birth, translated in French or English
- Canadian passport or permanent resident card (if the card has not yet been issued, a Record of Landing or Confirmation of Permanent Residence), all issued by Immigration, Refugees and Citizenship Canada
- Canadian citizenship certificate
You can also provide, among the following documents, two different documents on which the same date of birth is indicated:
- Québec health insurance card
- Passport from another country or copy of the passport's page with the photo and personal information
- Copy of the page of a Canadian passport with the photo and personal information
- Driver's licence accepted in Québec
Details on the format of the birth certificate
The
Directeur de l'état civil issues two formats of birth certificates: abbreviated format and long form. The same information appears on both formats of the certificate, except for the parent's names, which only appears on the long form. Therefore, when the name of the parents is a necessary information for processing the proof, we can require the birth certificate's long form.
