Reassessment of the health condition
You received an Application for Reassessment of the health condition
To verify wether your child's condition still meets the eligibility requirements for the Supplement for Handicapped Children and, if applicable, the Supplement for Handicapped Children Requiring Exceptional Care, Retraite Québec can, in certain cases, request a reassessement. The frequency of the reassessment varies according to each child's disability and situation. The Application for Reassessment ensures that the child's disabilities, limitations and/or the medical care he or she receives still entitle him or her to financial assistance, whether it is the Supplement for Handicapped Children or the Supplement for Handicapped Children Requiring Exceptional Care.
Complete the Reassessment Report you received by mail or the one that applies to your child's current condition. From the moment an Application for Reassessment has been sent to you, you have 120 days to return the completed form to us. If you have medical or school documents on hand (individual education plan, report cards, etc.), enclose them with your application.
While studying your child's file, with your signed consent, Retraite Québec will take steps to obtain additional documents from professionals in the health and education sectors, if necessary.
Important!
Please note that if you received an Application for Reassessment but that it is not completed or sent back, we will stop paying the Supplement for Handicapped Children the month following the deadline indicated for providing the application. However, you will continue receiving Family Allowance payments, even if the Supplement for Handicapped Children ceases to be paid.

You want to request a reassessment for the Supplement for Handicapped Children Requiring Exceptional Care
If the situation of a child already eligible for Tier 2 of the Supplement for Handicapped Children Requiring Exceptional Care has deteriorated or worsened over time, the child may become eligible for Tier 1 of the Supplement for Handicapped Children Requiring Exceptional Care. If you believe that this is the case for your child, we suggest that you file an Application for Reassessment. Retraite Québec will reassess your child's file to determine whether his or her current situation now meets the eligibility requirements for Tier 1 of the Supplement for Handicapped Children Requiring Exceptional Care.
